Losing speech after a stroke is frequently more distressing to a patient than losing movement, and it is far more often left untreated. Families adapt to guessing, the patient withdraws, and by the time therapy starts the isolation has done its own damage. Starting speech therapy early — even in small, frequent sessions — changes both the clinical outcome and the patient's mood.
Swallowing is the other half of this service and the more urgent one. Silent aspiration, where food or liquid enters the airway without any visible cough, is a leading cause of pneumonia in stroke and elderly patients. A bedside swallow assessment takes under an hour and often results in immediate, concrete changes: a thicker liquid consistency, an upright positioning rule, a slower feeding pace.
Sessions are delivered at home by qualified speech-language pathologists, with a parent or primary caregiver present throughout. Homework is short and specific — five minutes, several times a day, works far better than one long session nobody sustains.

About speech therapy
That pattern — intact comprehension with impaired expression — is common after a left-hemisphere stroke and is one of the more responsive presentations to structured therapy. The assessment will confirm the pattern before any therapy plan is written.
Coughing during or after meals, a wet or gurgly voice afterwards, meals taking far longer than they used to, or repeated chest infections. Any of these warrant a swallow assessment before the next hospital admission, not after it.
If a two-year-old has fewer than fifty words or is not combining two words, an assessment is worth doing. Early intervention is materially more effective than waiting to see, and an assessment costs you little if the answer turns out to be reassurance.
Yes, though gains are slower than in the first year. Chronic aphasia responds to intensive, functional practice — we set expectations honestly at assessment rather than selling an open-ended package.
